Disability.gov is the federal government website for comprehensive information about disability-related programs, services, policies, laws and regulations. The site links to thousands of resources from many different federal government agencies, as well as state and local governments and nonprofit organizations across the country.
Every day, new resources are added to Disability.gov’s 10 main subject areas: Benefits, Civil Rights, Community Life, Education, Emergency Preparedness, Employment, Health, Housing, Technology and Transportation. You’ll find information on topics such as Social Security disability benefits, job accommodations for employees with disabilities, accessible housing, and organizations in your state that can help you find a job or live independently.

The FosterClub Transition Toolkit can help foster youth and the adults supporting them develop a comprehensive transition plan. This Transition Toolkit is built around ten different domains (or topics): finances + money management; job + career; life skills; identity; permanence; education; self-care + health; housing; transportation; community; culture and social life. In this Toolkit, each of these domains is shown as a line on a subway map. The “subway” map will give you a look at everything you’ll need to plan for on your way to successful life on your own. The Toolkit also provides with transition planning worksheet templates.
A "Health Home" is not a physical place; it is a group of health care and service providers working together to make sure you get the care and services you need to stay healthy. Once you are enrolled in a Health Home, you will have a care manager that works with you to develop a care plan. A care plan maps out the services you need, to put you on the road to better health. NY Connects provides information and assistance related to long-term services and supports in New York State for people of all ages or with any type of disability. NY Connects uses a searchable directory for programs, providers, and services in your area.
NY Connects staff can help link you to long-term services and supports, such as home care, transportation and meals. The goal is to help individuals live as independently as possible while meeting their medical, social and functional needs that arise from aging or disability. This program helps individuals, families, caregivers, and professionals.

ONA Legal Counsels regularly hold legal consultation days at each ONA Opportunity Centers and can provide you with support for naturalization as well as other legal services.
The Liberty Defense Project (LDP) provides legal services to all immigrants, regardless of their status. LDP partners across New York State provide the following: Free legal services including consultations and direct representation for all immigrants, including those facing deportation; Urgent legal assistance through the LDP Regional Rapid Response program; Know Your Rights trainings; Law-related support to families of those who are in removal proceedings or have been deported.
The purpose of this handout is to assist people with disabilities to obtain an accurate and fair ADA paratransit eligibility determination. Many people with disabilities who should be eligible for paratransit services according to the Americans with Disabilities Act (ADA) are denied eligibility because transit agencies are not accurately assessing their capacities. People with disabilities can affect this process, in many cases, by carefully documenting their disabling conditions, ensuring all their disability-related impairments are assessed, and taking other steps as described on this website.
Patient AirLift Services is a nonprofit that arranges free flights for medical patients requiring medical diagnosis, treatment or follow-up who cannot afford or are unable to fly commercially.

PALS also arranges volunteer flights for family members of patients as compassionate missions, to ensure patients have support when they are away from home for long periods. PALS is proud to assist military personnel and their families with free flights to aid in the recovery and rehabilitation processes of our wounded veterans.
Finally, PALS has a history of supporting humanitarian efforts in the event of natural or man-made disasters.
Regional Interagency Technical Assistance Teams (RiTATs) meet regularly and use cross-systems collaboration to address system level issues and support the implementation of the System of Care core values and principles within their agency or organization. Family and youth serve as full partners to share the experiences and needs of families and youth served by the System of Care, and facilitate problem solving at the program and policy levels.
The Team serves as a liaison between the New York State-level senior staff members with regard to improving cross systems collaborations. The RiTAT is committed to sharing information and delivering quality technical assistance and consultation to agencies and counties which will enable them to achieve better outcomes for children, youth and families. As collaborative initiatives have grown at the state level, it became evident that leadership and resources at both the state and county level were necessary. Collective and shared knowledge and expertise of the team are used to maximize efforts, avoid duplication and help support evidence-based programming.
